Remove any accessories from the item being serviced such as knives, safety tubes, weights, regulator clips, etc. Big Island Divers is not responsible for any item not removed. If an item has become obsolete (parts no longer available) or if the cost of service would be prohibitively expensive, we will contact you before any work is done. Big Island Divers technicians are trained on Cressi, Atomic, Hollis, Oceanic, ScubaPro, Aqualung, Apeks, Tusa, Zeagle, Hog, Sherwood, Genesis and Edge brand equipment repair. Service Agreement: By signing this Service Agreement, I authorize Big Island Divers (BID) to perform the necessary services that I have chosen or have discussed with me. I recognize that the prices quoted are for service only and do not include parts such as hoses, mouthpieces, and batteries. I recognize that BID reserves the right not to service equipment that BID deems unsafe to operate. I will pay for all costs associated with the equipment listed and will forfeit any and all items left at BID for more than 90 days. I agree and authorize BID to conspicuously disable cylinders accepted for visual inspection if it does not meet standards published by PSI-PCI (Professional SCUBA Inspectors – Professional Cylinder Inspectors). I will receive a written report which will identify the reason(s) for failure. I understand that in this case I will have the disabled cylinder and all components returned to me. I will thoroughly inspect and test all equipment for safety and proper operations prior to each dive. I understand that if BID should be found liable for any injuries, damages and losses due to a failure to perform any or all its obligations or a failure of the equipment to properly operate, BID’s liability shall be limited to a sum equal to the total of the money paid to BID for services. This liability shall be exclusive and will also apply if gear is lost or damaged. I hereby state I carefully reviewed this Service Agreement and I acknowledge, recognize, and assume all risks inherent in the sport of scuba diving and in the use of scuba diving equipment. I waive each and every claim for injuries, damages and losses, including claims for negligence, which I may have against BID and its employees, instructors, agents and representatives, arising out of services performed or not performed under this Service Agreement. This Service Agreement shall be binding upon myself, heirs and legal representatives.
